 Introduction
Medicfusion is the service we use to securely store medical records.
Remember sitting in the office filling out all those papers regarding your health history, personal data and contact information? Guessing at the address and phone number of your doctors? Not sure exactly what your insurance number is? And then not finishing in time before your appointment? Now, with our new system that has all changed. Medical information is now being taken through the SECURE Medic Fusion web site. Your information and records are available only to health care providers approved by you. We care about your privacy and are confident about the security of your information with Medicfusion.
Your medical history and other related information IS required for you as our patient and once you dive in to the Medicfusion system and give it a try we hope that you will find it easy to work with. Most of this information you only need do once unless something changes - but keep your login info handy because you can easily verify your appointment times - both upcoming and past - right there on your personal web space! |
STEP 1: Register!
- Our office staff will give you your 4 digit PIN to get going. (help)
- With PIN in hand, go to: the Meidcfusion Web Site. This will take you to the patient registration area - see image at right, click to enlarge. (help)
- Next, simply enter your First Name, Last Name & PIN (Personal Identification Number). Then click "Continue"
- Follow the 5 simple steps (athough it's really just 2 steps + confirmation!):
- Tell us About yourself: (name, phone, etc.), your email address, create a password & a secuirty question; accept agreement
- Security Checkpoint: Accept Releases - available on the site or you may click here to download PDF of releases (version valid Sept 2010)
- Check your work: reivew of info in step 1&2.
- Complete: Welcome message ~ you have successfully registered!
- Activate: one last thing ==> go check your email and activate your account by clicking the link in the email from fountainchiropractic.info.
- You are now registered! Go to medicfusion.fountainchiropractic.info to login!
Note: you can always find a link to the login at the top right of every page of the web site!
Note: after successful registration you no longer need the PIN. Login will be accomplished with email & password

STEP 2: Login & Enter Medical Info
In step 1 above we registered. Now it's time to login & enter your medical information! This is your EHR (Electronic Health Record). The information is stored on a secure server by Medicfusion. It is safe and confidential.
- After you click the activation link in your email you will be directed to medicfusion.fountainchiropractic.info so that you can login with the email address & password you entered in Step 1, Item 4.1 above.

The first login will take you to a page that looks like this:
PINK areas indicate incomplete or out-of-date information.
- Please now click on each of those pink areas to update the information

You will need a secure PIN number (Personal Identification Number) and valid email address for registration. Please contact
Fountain Chiropractic at 360-379-9284 for your PIN#.
If you have already registered you will get the message:
This account has already been registered. Please login using your username and password. If you get this message but don't know what you have for a password you can reset your password - Click here to reset your password.
